CONSENT FOR EMPLOYMENT APPLICANTS

As the data subject, I hereby grant my voluntary consent to the processing, management and archiving of my personal data for the purpose of the selection of a suitable candidate and the subsequent conclusion of an employment or other similar relationship between the relevant person and GQsystems, s.r.o., Strojnícka 34, 821 05 Bratislava, Slovak Republic (operation where all the personal data are processed), Kasalova 518/27, 949 01 Nitra, Slovak Republic (the official registered office where no personal data are processed and no business activities performed), business ID No: 46 371 621, e-mail: office@gqsystems.eu, responsible person Filip Gergely (“GQsystems, s.r.o.” or “the Company”), in particular within the scope:

  • name, surname, title, date and place of birth, birth ID number,
  • permanent address, contact details (telephone number, fax number if relevant, and e-mail address),
  • education, language and computer skills, other courses/certificates/permits, employment data,
  • marital status, nationality, citizenship, photographs, video recordings, psychological tests, psychological-diagnostic tests

as well as special categories of personal data, and potentially other data presented in my curriculum vitae, employment application, motivational letter and other enclosed documents within the meaning of Act No 18/2018, on personal data protection, and Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation).

At the same time, I hereby confirm that the personal data I have presented are true. GQsystems, s.r.o., to which I am granting my consent, will not perform cross-border transfers of my personal data. The main purpose of the processing, management and archiving of my personal data is the pre-contractual relationships between myself, employment applicant, and GQsystems, s.r.o. for the filling of a vacancy. The conclusion of an employment relationship or similar relationship with GQsystems, s.r.o. is considered to be the fulfilment of the purpose of the processing of the personal data.

In the event the purpose of the processing of the personal data is not fulfilled within 6 months from the delivery of the data, all my personal data will be destroyed through shredding and deletion in accordance with statutory requirements and the internal guidelines of GQsystems, s.r.o. This deadline may be extended to 4 years with the consent of the applicant, which forms part of this document. Withdrawal of consent must be made in writing and must be delivered in writing to GQsystems, s.r.o. to the address of the company operation or by e-mail to office@gqsystems.eu. Such withdrawal of consent is effective on the date of delivery of the withdrawal to GQsystems, s.r.o.

The granting of this consent is without prejudice to my rights as the data subject. Based on written request or a request made in person at GQsystems, s.r.o. I have the right:

  • to request access to my personal data and for the correction, deletion or restriction of the processing of my personal data;
  • to object to the processing of my personal data;
  • to the portability of my personal data;
  • to submit a petition to initiate proceedings at the Office for Personal Data Protection of the Slovak Republic.

 

 

As the data subject, I declare that the personal data I have provided are true, up-to-date and voluntarily provided.

  • I consent and am aware that the Company will provide my personal data within the above scope to third parties – clients of the Company – in order to fulfil the above purpose, meaning to fill a vacancy. I am also aware that failure to consent to the provision of my personal data to third parties – clients of the Company – may result in frustration of the purpose of the selection procedure for filling such vacancy.

  • I consent to the processing, management and archiving of my personal data within the above scope for 4 years for the purpose of the selection of a suitable candidate and the subsequent conclusion of an employment relationship or similar relationship.
